Who this guide is for
This guide is for doctors considering ACCS Emergency Medicine ST1 through the 2027 national recruitment process. It explains how the MSRA fits with interview and ranking. The 2027 person specification and your Oriel advert determine your individual eligibility; this summary cannot replace either. The NHS England guidance covers England, Scotland and Wales and points applicants to separate Northern Ireland guidance. Official ACCS EM overview.

Check eligibility before applying
The 2027 ACCS EM ST1 person specification requires an eligible medical degree, full GMC registration at application, a licence to practise at post start, right to work in the UK and evidence of recent Foundation competences through a listed route. It also sets broader professional and specialty criteria, including judgement in the initial management of acutely ill patients. Read the full specification for exceptions, required supporting forms and your own circumstances. If CREST is needed, the 2027 applying guidance accepts the 2024 or 2027 version, subject to validity and the stated deadline. The office may check eligibility in phases, so progressing to MSRA or interview does not settle eligibility.
How the MSRA is used
Eligible 2027 ACCS EM applicants must sit the MSRA; the recruitment office will not accept a score from a previous sitting. If eligible applications exceed interview capacity, total MSRA score determines who is invited to interview. The office gives an approximate capacity of 882 interviews, not a fixed score threshold or guaranteed number. A 2027 numeric cut-off is TO BE CONFIRMED; do not use another specialty's threshold. ACCS EM applying guidance.
The central 2027 MSRA sitting notice says January is open to all specialties and February is reserved for applicants to GP and/or Core Psychiatry. An ACCS EM applicant must follow the allocated testing window in the actual invitation, especially if applying to multiple specialties. The MSRA structure page describes a Professional Dilemmas (PD) paper and a Clinical Problem Solving (CPS) paper delivered as one assessment. Begin with the official practice paper to learn both formats.
Interview, portfolio and final rank
The 2027 process uses a scored online interview. The applying page describes two stations of up to ten minutes each: a portfolio station and a clinical/ethical station. The interview page says interviews use Qpercom Recruit, that slots are booked through Oriel and that identity and technical checks apply. Prepare concise examples of personal contribution, teamwork, patient safety and commitment to emergency medicine; confirm any specific portfolio instructions in your invitation. The office has not published a separate portfolio upload timetable on the pages checked, so this is TO BE CONFIRMED.
The final rank combines a scaled MSRA contribution of 40% and the online interview contribution of 60%. This is a final selection weighting, not a raw MSRA mark or a pass score. Appointable applicants are matched to available posts based on rank and preferences; offers are made through Oriel. Scoring and offers.
What changed for 2027
The recruitment office says there is no longer a separate Core Training option, longlisting occurs in two phases, the 2024 or 2027 CREST can be accepted subject to its rules, and interview now has two ten-minute stations. These are specialty-specific changes from the 2027 applying page. Across specialty recruitment, the 2027 prioritisation guidance says priority status applies when offering interviews and training places for this route; it does not change an applicant's performance rank. The Round 1 application limit remains five. Check the central guidance for the exact categories and any review process rather than inferring priority from nationality or training history alone.
A practical preparation plan
First, verify eligibility and any CREST requirement. Next, divide MSRA revision between CPS clinical decisions and PD professional judgement, using the official practice paper to identify weak areas. Keep a short log of missed question themes and return to them. Alongside revision, prepare truthful evidence examples that you can explain in a short portfolio station, and practise clear clinical and ethical reasoning aloud. Review the current interview page and your Oriel messages as the selection window approaches. Try the free QBank demo, create a free practice account or compare optional membership plans if useful; these are not official recruitment resources and cannot guarantee a result. ACCS EM FAQs
Is MSRA mandatory?
Yes for eligible applicants in this 2027 process. Previous-sitting scores are not accepted. Official applying page.
Does 882 mean a score cut-off?
No. It is the office's approximate interview capacity. A numeric 2027 threshold is TO BE CONFIRMED.
Can I choose the February MSRA sitting?
The central notice reserves February for GP and/or Core Psychiatry applicants and says allocated windows cannot be changed. Follow your personal invitation; applying to ACCS EM alone gives no February entitlement. Central sitting notice.
Is the portfolio an extra component beyond the 60% interview?
