Key Points about Endometriosis:
- Definition: Endometriosis is a chronic inflammatory condition where endometrial-like tissue grows outside the uterus, commonly affecting the ovaries, fallopian tubes, and pelvic peritoneum. It is associated with pelvic pain and subfertility.
- Causes: The exact cause is unknown, but factors such as retrograde menstruation, immune system disorders, and genetic predisposition play a role. Hormonal imbalances that stimulate endometrial-like tissue growth are also implicated.
- Symptoms: Symptoms include chronic pelvic pain, dysmenorrhea (painful periods), dyspareunia (pain during intercourse), heavy menstrual bleeding, and infertility. Pain intensity does not always correlate with the extent of disease.
- Investigations: Diagnosis is often clinical but can be confirmed by laparoscopy. Imaging such as pelvic ultrasound or MRI may help assess the extent of endometriosis, particularly in cases involving ovarian endometriomas.
- Management: Treatment depends on symptoms and fertility desires. Options include pain management with NSAIDs, hormonal treatments (e.g., combined oral contraceptives, progestogens, or GnRH analogues), and surgical interventions such as laparoscopic excision or ablation of lesions. Assisted reproductive technologies may be needed for infertility.
- Complications: Complications can include chronic pain, adhesions, and infertility. Endometriosis can also impact mental health, leading to anxiety or depression due to chronic pain and the challenges associated with fertility.
