Key Points about Otitis externa:
- Definition: Otitis externa, commonly known as “swimmer’s ear,” is an inflammation of the external ear canal. It is often caused by bacterial or fungal infections.
- Causes: The most common causes of otitis externa are bacterial infections, particularly by Pseudomonas aeruginosa and Staphylococcus aureus. Excess moisture in the ear, trauma (e.g., from cotton swabs), and skin conditions like eczema can predispose individuals to otitis externa.
- Symptoms: Patients typically present with ear pain, itching, a feeling of fullness in the ear, and sometimes discharge. There may also be redness and swelling of the ear canal, and in severe cases, hearing may be temporarily reduced.
- Diagnosis: Diagnosis is clinical, based on the history of symptoms and otoscopic examination showing inflammation or infection of the external ear canal.
- Management: Treatment involves cleaning the ear canal and applying topical antibiotic or antifungal ear drops. Analgesics can be used to relieve pain. In some cases, ear wicks may be required to ensure that medications reach the deeper parts of the ear canal.
- Complications: Untreated or severe cases of otitis externa can lead to complications such as malignant otitis externa, particularly in immunocompromised individuals or those with diabetes, where the infection spreads to surrounding structures, including the bone.
