- Definition: A nasal septal haematoma is a collection of blood within the nasal septum, usually resulting from trauma. It occurs between the cartilage or bone of the septum and the mucoperichondrium or mucoperiosteum.
- Causes: The most common cause is nasal trauma, such as a sports injury or an accident. It can also occur post-surgery or in cases of spontaneous bleeding.
- Symptoms: Patients may present with nasal obstruction, pain, swelling, and sometimes nasal bleeding. The nasal septum may appear swollen and boggy upon examination, and bilateral haematomas can cause difficulty breathing through the nose.
- Diagnosis: Diagnosis is clinical, made through careful examination of the nasal cavity. Anterior rhinoscopy typically reveals a soft, fluctuant swelling on one or both sides of the septum.
- Management: Urgent surgical drainage is required to prevent complications such as cartilage necrosis, septal perforation, and saddle nose deformity. Broad-spectrum antibiotics are often prescribed to prevent secondary infection.
- Complications: If left untreated, the haematoma can lead to abscess formation, cartilage necrosis, and septal perforation. This can result in cosmetic deformities and chronic nasal obstruction.
