Introduction to Hoarseness

Key Points about Hoarseness:

  • Definition: Hoarseness is a change in the quality of the voice, often described as raspy, breathy, or weak. It occurs due to dysfunction of the vocal cords, which can be caused by various underlying conditions.
  • Causes: The common causes of hoarseness include acute laryngitis (often due to viral infections), vocal cord overuse (e.g., excessive talking or shouting), smoking, and gastroesophageal reflux disease (GERD). More serious causes include vocal cord nodules, polyps, or tumors (such as laryngeal cancer).
  • Symptoms: The hallmark symptom is a change in voice quality. Depending on the underlying cause, patients may also present with throat discomfort, cough, difficulty swallowing, or, in more severe cases, breathing difficulties.
  • Diagnosis: The diagnosis involves taking a detailed history and performing a physical examination. Laryngoscopy, which visualizes the vocal cords, is a key diagnostic tool. Further investigations may include imaging studies (e.g., CT or MRI) if a mass or more serious pathology is suspected.
  • Management: Treatment depends on the underlying cause. Resting the voice, managing contributing factors (e.g., reflux or smoking), and voice therapy are common approaches. In cases of vocal cord lesions or tumors, surgical intervention may be necessary.
  • Complications: If untreated, hoarseness caused by underlying conditions like cancer can progress to more serious complications such as airway obstruction or metastasis.
