Benign positional paroxysmal vertigo
Introduction to Benign positional paroxysmal vertigo
Welcome to the Benign positional paroxysmal vertigo subsection of the ENT for the MSRA course.
This section is designed to provide comprehensive and structured learning materials to help you understand and master the topic of Benign positional paroxysmal vertigo (BPPV) for the MSRA.
Key Points about Benign positional paroxysmal vertigo:
- Definition: BPPV is a disorder of the inner ear that causes brief episodes of vertigo triggered by changes in head position. It is the most common cause of peripheral vertigo.
- Causes: BPPV is caused by dislodged calcium carbonate crystals (otoconia) that move into the semicircular canals, leading to abnormal signals to the brain about head movement.
- Symptoms: Patients typically experience brief episodes of vertigo, often lasting less than a minute, brought on by changes in head position, such as rolling over in bed or looking up. Nausea may accompany vertigo, but hearing loss or tinnitus are generally absent.
- Diagnosis: The diagnosis is primarily clinical, based on characteristic history and positive results on positional tests such as the Dix-Hallpike maneuver, which triggers vertigo and nystagmus.
- Management: Treatment involves canalith repositioning maneuvers like the Epley maneuver, which aim to move the dislodged otoconia out of the semicircular canals. Most cases resolve after a few sessions. Medications are generally not indicated for BPPV.
- Complications: Though rare, untreated BPPV may lead to recurrent falls due to the sudden onset of vertigo, particularly in older patients.
